Home Care Association Honors Nassau County Organization for Exceptional Service
ALBANY, NY (06/08/2007)(readMedia)-- As part of their Annual Meeting and Conference, held June 10 – 12 in Saratoga Springs, NY, the Home Care Association will honor South Nassau Home Care’s Long Term Home Health Care Program for their outstanding contributions and service in the field of home care. The Home Care Association is pleased to honor the winners for their passion in providing care that is truly inspiring. Their tireless efforts and commitment to excellence are to be commended.
Patient Award
South Nassau Home Care’s Long Term Home Health Care Program on behalf of Mrs. V
This award recognizes a patient in home care who best exemplifies the ability to maintain a high quality of life while using home care services.
Mrs. V. has been a patient on South Nassau Home Care’s LTHHCP since 2002. She came with multiple health issues, including a debilitating stroke in 2002 at the age of 52 that left her a quadriplegic requiring around the clock care. Following her stroke, she remained in a nursing facility for many months, but after much education and planning, she was able to be cared for at home by her devoted husband. From the first visit, it was evident that this was an environment where the agency could assist in maintaining a good quality of life for this woman.
Mr. V. was capable and willing, indeed wanted, to care for his wife and only requested respite care during the day so that he could run errands and take care of personal business. Together with the help of a dedicated Department of Social Services case worker, South Nassau LTHHCP established a plan of care that met the patient’s needs and satisfied all safety concerns. The assigned aide has become a trusted and loved caregiver by the patient and her entire family.
This patient is truly a nursing home-level patient, but with the combined efforts of her husband and the entire home care team she is thriving at home. Mrs. V. leads a fulfilling life at home and her family, including children and grandchildren, visit often. She is a delight to the entire health care team and treats all like family. Being able to remain home with the people and things she loves has made this situation work. This is truly a “Nursing Home Without Walls” patient who continues to lead a meaningful and happy life at home.
