• Within three months of the date of this Order, shall submit to the Board his revised protocols and practices that relate to pharmaceutical storage, administration, destruction, and documentation, including descriptions of how these protocols and practices are enforced in the facilities where he serves as the medical director.
• Within six months following the date of this Order, shall successfully complete pre-approved coursework on ethics and professional conduct.
• Six months following submission of his revised protocols and practices to the Board, he shall submit to a random records audit for the facilities where he serves as medical director.

• Following completion of the above-referenced coursework, shall write and submit a paper, for review and approval by the Committee, discussing what he has learned from the coursework, how that knowledge applies to the facts of this case, and how this type of behavior will never happen again.
• No sooner than six months from the date of this Order, may petition for reinstatement of an unconditional license upon submission of satisfactory evidence of completion of the terms of this Order. Upon petitioning for reinstatement, shall be required to appear before the Committee to discuss his petition and practice plans. Upon hearing the petition, the Committee may recommend that the Board continue, modify, or remove the suspension or impose conditions and restrictions as deemed necessary.

• Upon submission of satisfactory evidence of completion of the terms of this Order, may petition for reinstatement of an unconditional license.

• Shall meet monthly with a supervising physician, who will be approved in advance by the Committee or its designee, to discuss his practice. The supervising physician shall provide quarterly reports to the Board or its designee regarding Dr. Said's overall work performance, including interpersonal communication with co-workers. He is responsible for ensuring timely submission of all required reports.
• The Order shall remain in effect for a minimum of two years. At the end of this period, may petition for reinstatement of an unconditional license. Upon hearing the petition, the Board may continue, modify, or remove the conditions on his medical license.
