4. He shall maintain records that reflect the patient complaint, clinical findings, treatment plan, response to treatment, and prescriptions authorized, including refills.

5. Six months following his successful completion of the pre-approved courses, he shall undergo an audit of his prescribing practices. After reviewing the results of the audit, the Committee may recommend further audits as it deems appropriate.

6. He shall develop, and submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions.

7. He shall develop, and submit for Committee approval, a narcotics agreement, to be used with all chronic pain patients.

8. He shall pay a civil penalty in the amount of $1,200.

9. He may petition for reinstatement of an unconditional license upon successful completion of the terms of the Order.

3. Following completion of the pre-approved coursework, he shall write and submit for Committee approval a paper, not to exceed five pages, reporting on how what he learned could be implemented to improve his practice.

4. He shall not engage in personal relationships with patients outside of the clinical setting that lack an objective focus on community purpose or benefit to a group, rather than of benefit to the individuals.

5. He shall provide clinical services only at places and times appropriate to the standards of the profession.

6. He shall not employ anyone who has been a patient in the previous twelve months. He may treat current employees in urgent or emergent situations, but must comply with the following conditions:

i. He must refer the patient to another provider within 48 hours.

ii. He may continue to treat the patient until care is assumed by another provider, or as long as the patient remains in an urgent or emergent situation, and.

iii. He must report treatment of the patient to his supervising physician within 48 hours of beginning treatment, and every 72 hours until the patient is transferred to another provider's care or treatment is discontinued.

7. He shall not provide to or accept gifts from patients.

8. He shall obtain a pre-approved supervising physician, with whom he shall meet monthly for the first twelve months of the Order, and quarterly thereafter. The supervising physician shall make quarterly reports to the Board.

9. He shall meet quarterly with a designated Board member.

10. He shall pay a civil penalty in the amount of $1,000.

11. This Order shall remain in effect for a minimum of two years.
