3. He shall complete, within one year, the following pre-approved courses:

i. Chronic pain management.

ii. Chemical dependency awareness.

iii. Medical records management.

iv. Boundaries.

4. He shall practice in a pre-approved setting.

5. He shall develop and submit for Committee approval a written protocol for the management and tracking of controlled substance prescriptions.

6. He shall develop and submit for Committee approval a narcotics agreement.

7. He shall write a paper for Committee approval detailing what he has learned from the readings and coursework and what he might have done differently in his treatment of the patient.

8. The Order shall remain in effect for a minimum of three years.
