2. Within 90 days of the date of this Order, he shall successfully complete the following pre-approved courses:
a. Electrocardiogram interpretation.
b. Family practice review.
3. Six months following complete of the pre-approved coursework, he shall undergo an audit of his prescribing practices.
4. He shall maintain a daily log of all controlled substance prescriptions. The log shall be made available for monthly review by his supervising physician and for quarterly review by his designated Board member.
5. He shall maintain patient records that reflect the patient complaint, examination, diagnosis, treatment plan, response to therapy, and medications prescribed.

1. He is reprimanded.
2. He is prohibited from treating chronic pain patients.
3. He is prohibited from prescribing Schedule II through V controlled substances.
4. He shall read the Model Policy for pain control published by the Federal of State Medical Boards and Responsible Opioid Prescribing, A Clinician's Guide, 2nd Edition, by Scott M. Fishman, M.D.
5. He shall successfully complete the following pre-approved courses:
a. Chemical dependency awareness.
b. Medical records management.
c. Boundaries.
d. Pharmacology.
6. No sooner than twelve months from the date of this Order and upon successful complete of the requirements in paragraphs 4 and 5, he may petition to resume prescribing Schedule II through V controlled substances.
7. Six months after he is authorized to resume prescribing controlled substances, he shall undergo an audit of his prescribing practices.
8. He shall maintain records that reflect the patient complaint, clinical findings, diagnosis, treatment plan, response to treatment, and authorized prescriptions, including refills.
9. Upon being authorized to resume prescribing controlled substances, he shall maintain a daily log of all controlled substance prescriptions. The log shall be available for monthly review by his supervising physician and for quarterly review by his designated Board member.
10. He shall develop, and submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions.
11. He shall practice in a pre-approved group setting.
12. He shall meet monthly with a pre-approved, on-site supervising physician who shall make quarterly reports to the Board.
13. He shall meet quarterly with a designated Board member.
14. He shall pay a civil penalty of $3,600.
15. This Order shall remain in effect for a minimum of three years.
