5. She shall sign releases allowing the Board to obtain her medical, mental health, evaluation, therapy, chemical dependency, or other records.

6. Upon petitioning for reinstatement, she shall appear before the Complaint Review Committee, which may recommend that the Board continue, modify, or remove the suspension or impose conditions and restrictions as deemed necessary.
