3. She shall not prescribe, administer, or dispense any prescription drug or drug sample for her own use of her family members' use or anyone with whom she does not have a formal patient relationship.

4. She shall not have access to controlled substances in the work place without supervision.

5. She shall not order opioids or any other controlled substances online.

6. She may petition for reinstatement of an unconditional license upon submission of proof, satisfactory to the Board, of a minimum of five years of documented, uninterrupted recovery.
