Action Taken: Dr. Sutley's license is reprimanded and suspended, but the suspension is stayed contingent upon Dr. Sutley's compliance with the following terms and conditions:
• Shall participate in the Health Professionals Services Program ("HPSP") and fully comply with all terms and conditions of her HPSP Participation Agreement. Shall sign releases allowing HPSP to provide a copy of her Participation Agreement and all compliance and treatment data to the Board. A violation of her HPSP Participation Agreement shall constitute a violation of this Order.
• Within three months of the date of the Order, shall complete a comprehensive substance use and mental health evaluation by an evaluator preapproved by the Committee. Within ten days of notification of approval, Respondent shall schedule the evaluation. Respondent shall permit the Board to share private data with the evaluator. When the evaluation is complete, shall provide the evaluation to HPSP immediately.
• Shall sign all releases allowing the Board to obtain medical, mental health, and chemical dependency records from any treating professional, evaluator, or facility.
• May petition for reinstatement of an unconditional license no sooner than four years from the date of the Order. Upon petitioning, shall submit evidence satisfactory to the Board, of a minimum of four years of documented, uninterrupted recovery and mental health stability commencing no sooner than the date this Order.
