3. He may petition for reinstatement of his license upon submission of satisfactory evidence of at least three months of documented, uninterrupted recovery.

4. Upon petitioning for reinstatement, he shall appear before the Complaint Review Committee to discuss his petition.

5. He shall sign releases allowing the Board access to all medical, mental health, evaluation, therapy, chemical dependency, or other records from any treating health professional, evaluator, or facility.
