1. His license is suspended.
2. During the suspension, he shall not practice respiratory therapy in Minnesota and may not use any title indicating or implying that his is licensed as a respiratory therapist.
3. He may petition for reinstatement of his license upon submission of satisfactory evidence of at least 12 months of uninterrupted sobriety and at least 12 months of stable mental health.
4. Upon petitioning for reinstatement, he shall appear before the Complaint Review Committee, at which time the Committee may recommend that the Board continue, modify, or remove the suspension, or impose conditions or restrictions.
5. He shall releases authorizing the Board to obtain his treatment records.

Nature of Misconduct: Unprofessional and unethical conduct, improper management of medical records, and inappropriate prescribing practices.
Action Taken: Dr. Lewis' license is conditioned and restricted as follows:
1. He shall read the "Model Policy for the Use of Controlled Substances for Treatment of Pain," adopted as policy by the House of Delegates of the Federation of Statement Medical Boards. He shall also read "Responsible Opioid Prescribing, a Clinician's Guide," Second Edition. After completing the reading, and within three months of the date of this Order, he shall write and submit a paper discussing what he has learned and how he has implemented this knowledge into his practice.
2. He shall successfully complete, within one year, the following pre-approved courses:
a. Medical records management;
b. Electrocardiogram interpretation;
c. Chemical dependency awareness;
d. Professional boundaries;
e. Family practice review.
3. Six months following completion of the coursework, he shall undergo an audit of his prescribing practices.
4. He shall maintain a daily log of all controlled substance prescriptions.
5. He shall maintain patient records that reflect the patient complaint, examination, diagnosis, treatment plan, response to therapy, and medications prescribed, including refills.
6. He shall develop and submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions.
7. He shall develop and submit for Committee approval, a narcotic contract for use with all patients who receive narcotics for chronic pain.
8. He shall practice in a pre-approved group setting.
9. He shall establish a relationship with a pre-approved educational preceptor in family medicine. The preceptor shall submit quarterly reports to the Board.
10. He shall meet with a pre-approved supervising physician who shall submit quarterly reports to the Board.
11. He shall meet quarterly with a designated Board member.
12. This Order shall remain in effect for a minimum of two years after which he may petition for reinstatement of an unconditional license.
