Action Taken: Dr. Velasco's license is conditioned as follows:
1. He is reprimanded;
2. He shall successfully complete, within one year from the date of this Order, the following pre-approved courses;
a. Chemical dependency awareness,
b. Chronic pain management,
c. Medical records management, and
d. Boundaries;
3. Upon completion of the required coursework, he shall write and submit for Committee approval, a paper detailing what he has learned and how he will implement his new knowledge into his practice;
4. He shall maintain a daily log of all controlled substance prescriptions;
5. He shall maintain records that reflect the patient complaint, clinical findings, diagnosis, treatment plan, response to treatment, and prescriptions authorized, including refills;
6. Six months following his successful completion of the pre-approved courses, and as deemed necessary by the Committee, he shall undergo an audit of his prescribing practices;
7. He shall develop, and submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions;
8. He shall develop, and submit for Committee approval, a pain management contract;
9. He shall notify the Board of any change in employment;
10. He shall meet monthly with a pre-approved supervising physician who shall submit quarterly reports to the Board;
11. He shall meet quarterly with a designated Board member;
12. He shall pay a civil penalty in the amount of $6,950.40;
13. This Order shall remain in effect for a minimum of two years.

Action Taken: Dr. Wasemiller's license is suspended. The suspension is stayed contingent upon his compliance with the following conditions:
1. He is reprimanded;
2. He shall successfully complete, within two years, the following pre-approved courses:
a. Family practice review,
b. Pain management,
c. Chemical dependency awareness,
d. Medical records documentation;
3. Following completion of the pre-approved coursework, he shall undergo a practice audit;
4. He shall develop, and submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions. He shall implement this approved protocol in his practice;
5. He shall keep a daily log of all controlled substance prescriptions he approves;
6. He shall practice in a pre-approved group setting;
7. Shall meet monthly with a pre-approved supervising physician who shall review his daily controlled substance prescription logs. The supervising physician shall submit quarterly reports to the Board;
8. He shall meet quarterly with a designated Board member/reports;
9. He shall pay a civil penalty in the amount of $4,662.10; and
10. The Order shall remain in effect for a minimum of three years.
