1. He shall read the Model Policy for pain control published by the Federation of State Medical Boards and Responsible Opioid Prescribing, A Clinician's Guide, Second Edition, by Scott M. Fishman, M.D..
2. He shall complete, within one year, the following pre-approved courses:
a. Chronic pain management,
b. Chemical dependency awareness,
c. Medical records management, and
d. Boundaries.
3. Following completion of the required readings and pre-approved coursework he shall write, and submit for Committee approval, a paper describing what he has learned and how he has implemented this knowledge into his practice.
4. Six months following his successful completion of the pre-approved courses, he shall undergo an audit of his prescribing practices. After reviewing the results of the chart audit, the Committee may recommend further audits as it deems appropriate.
5. He shall maintain records that reflect the patient complaint, clinical findings, treatment plan, response to treatment, and prescriptions authorized, including refills.
6. He shall submit for Committee approval, a written protocol for the management and tracking of controlled substance prescriptions, including use of the Prescription Monitoring Program.
7. He shall submit for Committee approval a narcotics agreement.
8. He shall pay a civil penalty of $500, and
9. He may petition for an unconditional license upon successful completion of the terms of this Order.

8. He shall meet on a quarterly basis with a designated Board member.
9. He shall pay a civil penalty in the amount of $3,563.20. and
10. This order is to remain in effect for a minimum of three years.
