Nature of Misconduct: Unprofessional and unethical conduct; improper management of medical records; prescribing a drug for other than medically accepted purposes.

Action Taken: Dr. Donovan's license is conditioned as follows:

  1. She is reprimanded.
  2. She shall successfully complete, within nine months, the following pre-approved courses:
    1. Medical records management;
    2. Pharmacology related to pediatrics; and
    3. Management of pediatric respiratory diseases.
  3. She shall successfully complete, within one year, a pre-approved comprehensive obstetrical review course.
  4. Within six months of her completion of the comprehensive obstetrical review course, she shall undergo an audit of her obstetrical cases.
  5. She shall meet monthly with a pre-approved supervising physician, who shall submit quarterly reports to the Board.
  6. She shall pay a civil penalty of $3,168 within six months.
  7. This Order shall remain in effect for a minimum of two years, after which she may petition for reinstatement of an unconditional license.

Action Taken: Dr. Johnson's license is conditioned and restricted as follows:

  1. She is reprimanded.
  2. She shall successfully complete, within six months, the following pre-approved courses:
    1. Chemical dependency awareness; and
    2. Professional boundaries.
  3. She shall maintain a daily log of all controlled substance prescriptions. The log shall be made available for weekly review by her supervising physician and for quarterly review by her designated Board member.
  4. Six months after her prescribing privileges are reinstated by her employer, she shall undergo an audit of her prescribing practice.
  5. She shall practice in a group setting and any changes to her group practice setting shall be approved by the Board.
  6. She shall meet monthly with a pre-approved supervising physician who shall submit quarterly reports to the Board.
  7. She shall meet quarterly with a designated Board member.
  8. She shall pay costs of $2,750 within six months.
  9. This Order shall remain in effect for a minimum of one year after which she may petition for reinstatement of an unconditional license.
