3. Within six months of successful completion of the pre-approved courses, he shall write and submit for approval a paper describing what he has learned from the medical records and professional boundaries courses, from the previously completed courses on chronic pain management and chemical dependency awareness courses, from the reading of the Federation of State Medical Boards' Model Policy on the use of Opioid Analgesics in the Treatment of Chronic Pain and Responsible Opioid Prescribing, by Scott M. Fishman, M.D., and how he has implemented this knowledge into his practice.

4. He shall maintain records that reflect the patient complaint, clinical findings, treatment plan, response to treatment, and the prescriptions authorized, including refills.

5. Six months following successful completion of the above-referenced coursework, he shall undergo a random audit of his patient charts.

6. He shall practice in a pre-approved setting.

7. He shall submit approval, a written protocol for the management and tracking of controlled substance prescriptions.

8. He shall develop and submit for approval, a written controlled substance agreement for use with all patients receiving controlled substances.

9. He shall pay a civil penalty of $4,004.04.

10. He may petition for reinstatement of an unconditional upon completion of the terms of this Stipulation and Order.
