Community Care Physicians, P.C. and CapitalCare Medical Group, LLC to Announce Consolidation

LATHAM, NY (02/09/2017) (readMedia)-- Community Care Physicians, P.C. and CapitalCare Medical Group, LLC will hold a press conference tomorrow, February 10th, at 10:00 am to announce their intent to consolidate. Both organizations share a vision for the future of healthcare and find that a clinically integrated, independent medical system is the key to achieve the quadruple aim; to improve the patient experience, the health of the population and employees, and to reduce the total cost of care.

Community Care Physicians and CapitalCare Medical Group are multispecialty medical groups that have been longstanding independent medical group practices in this community.
