Multispecialty Medical Facility in Clifton Park is Now Open

Community Care Physicians, P.C. is now seeing patients in the 60,000 square-foot Medical Arts Complex on Route 9

CLIFTON PARK, NY (09/22/2016)(readMedia)-- Community Care Physicians, P.C. (CCP) is now seeing patients in the new two-story medical arts complex located at 1783 Route 9, Clifton Park. CCP brought 11 different medical specialties together into a central location so patients can better access key medical services and physicians can better collaborate. Patients can see any of CCP's 40 providers with the added convenience of "one-stop-shopping", including primary care and specialty care, under one roof.

CCP is the primary tenant, occupying 45,000 square feet of the $15 million building, which was built by BBL Medical Facilities and is owned by Columbia Development Cos. Approximately 100 CCP employees are based in the new facility. In the building, CCP offers family medicine, internal medicine, pediatrics, ob/gyn, medical imaging, child neurology, physical therapy, urology, and lab. Audiology will open later this month and Urgent Care will open late Fall for walk in care nights, weekends and holidays. A gym will also be available on-site in the future.

"CCP will be able to care for patients from conception to end of life and improve our patients' experiences," said Dr. Miguel Diaz, family physician at Community Care Family Medicine of CCP. "This facility allows us quicker internal referrals, labs, imaging and process times and CCP patients can go from one department to another without having to leave the building."

All CCP providers and staff are connected through the electronic medical record, which allows for the smooth transfer of clinical information and efficient patient flows. Patients can also connect with the physicians electronically using an online, secure patient portal.

The new medical building offers patients onsite access to amenities including iPad stations in patient waiting areas and a concierge to assist patients with questions and directions. "Everything within this building was designed with our patients in mind," said Dr. Diaz.

Community Care's model is to create improved access to care through the integration of multiple specialties in one location. Columbia and BBL most recently built a similar medical office building for Community Care at the Rensselaer Technology Park in North Greenbush. CCP has other consolidated locations in Delmar, Latham, and Niskayuna.

The following CCP practices relocated from nearby locations in Clifton Park to the new consolidated building: Clifton Park Family Medicine, Clifton Park Pediatrics, Community Care Family Medicine, Community Care Internal Medicine – Clifton Park, The Office of Dr. Kathleen Downey, Capital Region Women's Care, Urological Institute of Northeastern New York, Community Care Physical Therapy. The following CCP practices opened new locations in Clifton Park, where they weren't currently operating: ImageCare Medical Imaging, LabCare, The Audiology Center, Urgent Care Center of Clifton Park (coming soon).

Also in the facility are Albany Medical Center and Brass Eye Center.

Community Care Physicians is holding a free community event, Movie in "The Park", on Friday September 30th to showcase the new building.
